To me it means running with Sensitivity high and discrimination as low as possible.  Its the sensitivity most people mean .

It can depend on which machine you are referring too. as Taff has said it can mean to running the detector with the sensitivity set higher than is recommended or more  traditionally it means that you manual ground balance then reduce it on the machine which makes it slightly more sensitive.
